Diabetes Prevention Patrol is Self Health Advocates who work one on one with you and your healthcare team to provide better care for you. They are the connector to your healthcare team.
Self Health Advocates are Resource Navigators, who connect people to resources in their community to help improve health.
We work to connect individuals to:
· Medical health, dental, or vision care
· Resources to help manage finances.
· Resources for healthy food choices.

The Diabetes Prevention Program Education Fund is a endowment that allow us to partner with Diabetes Education & Self Management Alliance to offer the National Diabetes Prevention Program to indigent individuals in our communities.
The Diabetes Prevention Program encourages anyone who desire to prevent diabetes to enroll in this program which is designed to aid in removing barriers to wellness and a healthy lifestyle for participants.
Participants in the program are able to identify chronic conditions and unmet health needs and even prevent diabetes.
Diabetes Education & Self Management Alliance is our Wellness Partners, who serve as the integrator organization for the National Diabetes Prevention Program.
The organizations provide their time, talent, and energy to make the program available at no cost to most individuals.

The Emergency Preparedness Coalition is a project funded by grants, donations and volunteers. We meet regularly to make public health emergency, disaster plans and preparations for Tangipahoa Parish residents.
It's important for individuals who are diabetics to remain prepared for emergencies and disasters, especially in rural areas.
The vision for this project was inspired by Faith Gurerra; an EMT at New Orleans Emergency Medical Services. Faith is a diabetic and during a major disaster in Louisiana, she lost access to her insulin.
Faith and I talked about it in detail and we utilized that experience to develop the DEPC Project. Today, we are working to help Diabetics during disasters.
